Garage door cables

hi, my garage door left and right cables both came off the drum 2 times in one day. Also i notice both left and right torsion springs came unwound twice also .the second time i winded the springs i put extra tight on the setting screws but they came unwound again . So i couldnt figure why would they become unwound if i made sure the setting screws are extra tight. Could it be the drums are bad ? The drums set screws seem fine they thighten good. Im so stump why would the spring unwind?
