Making a Folding table top

Hi,
I have a wall mounted table, and I want to make a foldable extension to this tabletop. something like this but without leg support, and rectangle in shape.
Thickness of plyboard is 12 mm and I have another one like that , my problem is that I haven't done thing like this in past and intuitively it seems that using a long hinge or regular hinges wont hold the weight of plyboard itself let alone weight of the computers , books etc.
so how can I implement this contraption ?
I am thinking of a custom hinge placed horizontally on table, combining both mounted tabletop and the extension,

but that is not very handy & easy to come by and it will probably look very ugly. I am looking for viable options, apart from buying a bigger table .

If it is possible, look at motor homes and campers. Almost all have folding tables and counter tops. Many are quite ingenious. If you have an RV supply shop near you they may be able to help too.
