How to fix leaky bathtub faucet handle?

Our shower tub hot water handle has a leaky spray. It's ok when the water is on for the tub, BUT once the shower is turned on then hot water sprays out of the handle fixture. I removed the handle cap and saw no washers (checked the cold handle too and no washer present). Looks like a leak in the fixture itself somehow.
